Coordinated Access (Intake) refers to a standardized process for determining medical vulnerability and eligibility for homeless assistance services. Administered by Nancy Carpenter, The Haven offers Coordinated Access appointments seven days a week, 365 days a year. To date, more than 1,200 intakes have been completed.

This detailed interview contains sensitive questions. To be most effective, the Coordinated Access should be completed by a client's case manager. Case Managers are encouraged to print the assessment from TJACH's website, and drop off completed intakes at The Haven, to the attention of Nancy Carpenter, anytime we're open. For those who do not have a dedicated case manager, The Haven offers Coordinated Access to individuals and families every day of the week. To schedule an appointment, leave a message at 434-973-1234 ext 113. Nancy Carpenter is The Haven's full-time Intake Coordinator.
