Coordinated Access

Coordinated Access (Intake) refers to a standardized process for determining medical vulnerability and eligibility for homeless assistance services. Administered by Nancy Carpenter, The Haven offers Coordinated Access appointments seven days a week, 365 days a year. To date, more than 1,200 intakes have been completed.

This detailed interview contains sensitive questions. To be most effective, the Coordinated Access should be completed by a client's case manager. Case Managers are encouraged to print the assessment from TJACH's website, and drop off completed intakes at The Haven, to the attention of Nancy Carpenter, anytime we're open.
